Please note that Simpson will no longer distribute the 1095 IRS forms effective for Tax Year 2025 in accordance with the Paperwork Burden Reduction Act.

You are able to request a form by logging into the benefits portal at: https://app.unifyhr.com/ and select the option to submit a request for a copy of your form.
