Leave of Absence

For Work Related Injuries, Please contact your Supervisor Immediately and Refer to our Worker’s Compensation Section Below.

​​​​​​​​​​​​​Simpson Manufacturing Co., Inc. has contracted with Matrix Absence Mgt. to administer & track its employees’ Family & Medical leaves.

You may qualify for protection under the Family Medical Leave Act of 1993 (FMLA) if you are absent:

  • Birth/Placement
  • Family Care​​
  • Employee Medical
  • Injured Service Member
  • Qualifying Exigency

You become eligible after one-year of service and 1250 hours worked during the 12 months before the beginning of the leave.

If you need to take a Family Medical Leave, please contact your branch HR for more information.

FMLA Employee Responsibilities:

  • Inform your supervisor when you need to be absent from work
  • When leave is foreseeable, provide 30 days notice prior to leave
  • Complete the FMLA form, have it signed by the appropriate health care provider, and return it to Matrix. Notification will be mailed to you regarding leave approval or denial
 
California Employee Responsibilities:
 
Washington State Employee Responsibilities:
  • ​For Washington employees taking a Paid Family leave or disability for your own illness, you must apply with the sate of Washington for benefits.  Refer to the Quick Links section to apply.

 

What Should I Do When I’m Able to Return to Work?

When returning to work from a FMLA absence for your own serious health condition, obtain medical provider’s authorization to return to work.

  • Bring your completed return-to-work authorization form to your supervisor
  • Call 1-877-202-0055 to provide information on your return to work plans​

 

Worker’s Compensation – What Should I Do If I’m Injured on the Job?

  • Notify your branch Supervisor and EHS Coordinator immediately.
  • Branch HR/EHS team only – Click here to fill out the Worker’s Compensation form or refer to useful documents section.

  • For employees outside of California and Ohio, WC claims are maintained by Liberty Mutual, and if there is not a designated facility, the Workers Comp Provider Referral Services can be found here​
  • For California and Ohio employees, WC claims are maintained by Tristar, and if there is not a designated facility, employees can get treated at the nearest urgent care, and Tristar will refer you elsewhere once the claim is initiated.
