1095 IRS Form Requests
Please note that Simpson will no longer distribute the 1095 IRS forms effective for Tax Year 2025 in accordance with the Paperwork Burden Reduction Act.
You are able to request a form by logging into the benefits portal at: https://app.unifyhr.com/ and select the option to submit a request for a copy of your form.
You can also request a form directly from Unify HR: https://benefits.strongtie.com/documents/1095-request-form.pdf/
